Referring Physicians

It is very easy to refer patients with potential sleep disorders who need a consultation. We have one centralized phone line for all 13 physicians: 704-510-9900. If it is more convenient we also have a fax number where you can fax the referral form to us and we will immediately contact the patient for scheduling purposes: 704-510-9918. At Carolinas Sleep Services we try our very best to make the process as simple as possible for the referring physician as well as the patient.
